This lecture provides a comprehensive evaluation of the rapidly expanding landscape of Interventional Psychiatry, focusing on circuit-based neuromodulation and advanced pharmacotherapies when traditional medications fail to address treatment-resistant conditions. The presentation systematically reviews the clinical indications, efficacy, safety profiles, and mechanisms of action for established modalities—including Electroconvulsive Therapy (ECT), Repetitive Transcranial Magnetic Stimulation (rTMS), intravenous ketamine, and intranasal esketamine—alongside emerging technologies like Prism neurofeedback, Vagus Nerve Stimulation (VNS), and Newer at-home devices. Dr. Cochran highlights the critical paradigm shift toward integrating these procedural interventions earlier into treatment algorithms rather than treating them strictly as a last resort.

Learning Objectives

  • Analyze and contrast the clinical indications, response rates, and safety profiles of ECT, rTMS, IV ketamine, and intranasal esketamine to appropriately select interventions for patients with treatment-resistant depression (TRD) and comorbid conditions.
