The presentation systematically reviews the clinical indications, efficacy, safety profiles, and mechanisms of action for established modalities—including Electroconvulsive Therapy (ECT), Repetitive Transcranial Magnetic Stimulation (rTMS), intravenous ketamine, and intranasal esketamine—alongside emerging technologies like Prism neurofeedback, Vagus Nerve Stimulation (VNS), and Newer at-home devices. Dr. Cochran highlights the critical paradigm shift toward integrating these procedural interventions earlier into treatment algorithms rather than treating them strictly as a last resort.

Learning Objectives

  • Analyze and contrast the clinical indications, response rates, and safety profiles of ECT, rTMS, IV ketamine, and intranasal esketamine to appropriately select interventions for patients with treatment-resistant depression (TRD) and comorbid conditions.
  • Formulate an early-intervention treatment algorithm that integrates neuromodulation techniques (such as rTMS augmentation) prior to extensive, late-stage medication switching protocols based on recent clinical evidence.
  • Review monitoring and safety protocols for patients undergoing neurostimulation or rapid-acting pharmacotherapies, ensuring appropriate screening for contraindications, management of acute side effects, and integration of post-treatment continuity of care.

  • Michelle Cochran, MD, DLFAPA, FCTMSS

    Dr. Michelle Cochran is a board-certified, award-winning psychiatrist specializing in interventional psychiatry and neuromodulation. Medically trained at University of Louisville School of Medicine, she chose psychiatry to make a tangible difference in patients' lives. She identified the transformative potential of transcranial magnetic stimulation (TMS) in 2008 and immersed herself in learning the early research, and became an early adopter of the technology. Her expertise and advocacy culminated in her leadership as president of the Clinical TMS Society, where she co-founded the PULSES international certification course, establishing a global standard for training which she directed in over twenty-one venues worldwide. Dr. Cochran has been honored to be inducted as a Fellow in the Clinical TMS Society (FCTMSS) for her leadership and work in the society. She is active in the Tennessee Psychiatric Association as the upcoming President, and in the American Psychiatric Association (APA) serving on its Assembly. She has received the APA Distinguished Lifetime Fellow designation (DLFAPA), and has been appointed the Chair of the Caucus on Neuromodulation.
